New Patient Information
Now that you are scheduled for your appointment, you will receive an email asking you to activate your patient portal. The subject line will read: Welcome to Your Skin of Culture and Hair Center Patient Portal (welcome@practicemailer.com). Please check your inbox and spam folder. Once you activate your account, you can use your patient portal by going to socah.ema.md and clicking “patient login”. You can also choose to download the PocketPatient(tm) App to access the portal on your mobile device. After you activate your portal, you will be able to communicate directly with our team regarding appointments, general information, clinical information, and much more! You will also be able to request an appointment, refills, and review financial information, as well as make payments, sign consents and waivers, and check in for your upcoming appointment! Checking in for your upcoming appointment after you have signed your consents and made a payment of your estimated responsibility (if any) helps us to get you seen quicker. All consents must be signed, pertinent medical information collected and updated, and payment collected (if any) before we can get you in to your appointment.
If you are a cash patient there is a required $50 non-refundable deposit to hold your appointment. The $50 is deducted from your office visit. Please note initial visits for skin concerns is $250 and for hair loss concerns is $250 and all follow-up appointments are $150 (no deposit required for self-pay follow up patients). When you arrive for your appointment the balance is due at that time. If you are unable to make your appointment on time we do offer all patients a 5 Min grace period. If you are unable to make it at your appointment time you may be asked to reschedule or change to a virtual visit. Same day cancellation fee is $50, and reschedule fee is $25 (must be paid before scheduling another appointment). If you need to change your appointment, please give us at least 24-hour notice.
You will receive “check in” email notification 48 hours before your appointment. Please take a moment to complete your intake forms in full, sign consents, and make a payment (if any), BEFORE you come in the office to avoid delaying or rescheduling your appointment.
Preparing For A Hair Loss Visit
Dr. Hill understands that every detail in the history and exam is crucial when assessing for hair loss. Please do not wash hair for 48 hours prior to your initial visit. Additionally, please remove any sew-ins, lace-fronts, braids, extensions, and other “glued-in, braided-in, sewed-in, or semi-permanent” styles so Dr. Hill can thoroughly assess the scalp. Lastly, please do not apply any camouflage products on the day of the exam (48 hours if possible) as this may affect visualization of the scalp. Please bring any biopsies and medications used in the past or be prepared to provide provider information to obtain this information if possible.
Preparing For A Full Body Skin Exam or Skin Care Exam
Please arrive with no make-up on or be prepared to remove the make-up so Dr. Hill can thoroughly evaluate your skin including your face.
Preparing For A Nail Exam
Please keep nails free from nail polish for 2 days prior to the appointment so no staining is visible and nails can be assessed for texture, appearance, and color.
I Have Had Treatments In The Past. Will Dr. Hill Need Those Records?
Dr. Hill wants to ensure she is not repeating any treatments you may have used in the past. Obtaining records from your previous physicians is helpful.
